Applying For Social Security Disability And Ssi Benefits
You can apply online for Social Security Disability benefits, you can file in person at your local Social Security office, or by calling 772-1213.

Before you begin your application, you should collect certain information regarding your medical treatment, work history and other facts. To view a checklist of the information that you will need, Disability Checklist.
When you are ready to apply for Social Security Disability Benefits, Apply for Disability. You will not have to complete the entire application in one sitting. You will have the option of saving your application and returning to it later.
You can also complete most of your application for SSI Disability benefits online, but to finish the application process you will have to visit a Social Security office. You canApply for SSI to begin the SSI application process for yourself or for a child.
Applying For Social Security Disability Benefits
Once you have gathered the necessary information, you should contact the SSA to complete a disability application. The actual application can be filled out either at the Social Security office, over the phone or online.
When submitting your application for Social Security Disability benefits, you will need to complete the Social Security Adult Disability Report.
This report contains eleven separate sections and asks for information about your medical conditions, any work activity you may be performing, your education and training, medical treatments you have undergone, medications you are taking and other disability-related information.
Once the SSA receives your application for Social Security Disability benefits and all of the documentation required, they will begin to review your claim. You may, at this point, be asked to go for a consultative exam. This exam is paid for by the SSA.
The purpose of the exam is to evaluate the extent of your disability and whether or not your condition meets the Social Security Disability guidelines. It is very important that you show up for your exam.
Not undergoing the required consultative exam may result in a denial of your Social Security Disability benefits. While the findings of the consultative exam do play a part in the decision the SSA makes in regards to your disability application, statements from your treating physicians and objective medical evidence are given more weight in the determination process.

The Basics About Disability Benefits
The SSDI program pays benefits to you and certain if you are insured. This means that you worked long enough and recently enough – and paid Social Security taxes on your earnings. The SSI program pays benefits to adults and children who meet our requirements for a qualifying disability and have limited income and resources.

What You Need For Apply For Ssdi Or Ssi Benefits
You will get a faster decision when you apply for Social Security Disability Insurance or Supplemental Security Income with the Social Security Administration if you provide:
- Your Social Security number
- Medical records from your doctors, therapists, hospitals, clinics and caseworkers
- Laboratory and test results
- Names, addresses, phone and fax numbers of your doctors, clinics and hospitals
- Names of all medications you are taking
- Names of your employers and job duties for the last 15 years

If You’re Not Sure Why You Received A Payment
If you receive a check or direct deposit payment from the Treasury Department and do not know what its for, contact the regional financial center that issued it. Only the agency that authorized the payment can explain why you received it.
If you received a check, look for the RFCs city and state at the top center. Then contact that RFC to find out which federal agency authorized the payment. It will be one of these:

Applying For Disability: Ssi And Ssdi

Social Security Disability Benefits: An Overview
The Social Security Administration pays disability benefits under two programs:
Social Security Disability Insurance SSDI provides disability benefits to blind or disabled individuals who are insured based on federal insurance contributions paid into the Social Security Trust Fund. Certain family members can also receive benefits based on a worker’s contributions.Supplemental Security Income SSI provides benefits to individuals who are disabled, blind or elderly, with limited income and resources, including children under age 18, who are disabled, blind or elderly.

However, if your health has more gradually declined over a period of time, it might be more difficult to figure out a specific date. In that situation, it will make more sense to put down the last day you worked or the day you made changes to your work routine. The date you settle on needs to be related to when you became unable to work. Do keep in mind that this date can be changed later in your case if it needs to be.

Are You Eligible To Apply For Benefits
Anyone is able to apply. Your local Social Security office staff will review the non-medical portion of your application to determine the benefits for which you are eligible. They will look at your work history, your age, your income and your resources.
In Oklahoma, your application is then sent to the Disability Determination Services . The DDS, a division of the Oklahoma Department of Rehabilitation Services, is responsible for making timely, and accurate disability determinations in accordance with Social Security Rules and Regulations.
DDS personnel, consisting of a physician or psychologist and a disability examiner, will consider all the facts in your case using medical information from your doctors, hospitals, clinics, and other places where you have been treated to determine if you are found to be medically disabled.
Once a determination on your claim is reached, you will get a written notice from the Social Security Administration explaining the determination. If your claim is approved, the notice will show the amount of your benefit and when payments will start.
