55 year old man injured both hands and arms while performing repetitive job duties at his place of employment. Our clients initial medical treatment consisted of two cortisone injections. He was then referred to an orthopedist. He was diagnosed with a bicep tendon rupture and posterior interosseous nerve syndrome. An MRI later showed a complete rupture of the biceps tendon. After this diagnosis he received a cortisone injection with Lidocaine to alleviate the inflammation. He experienced pain in both arms that spread to his hands and all of which kept him awake at night. He was then referred to a hand surgeon. The hand surgeon diagnosed tennis elbow and radial tunnel syndrome, and for which he prescribed acupuncture therapy. Decompression of the nerve on the right interosseous nerve was performed by the surgeon. Scar tissue was also removed. The surgeon gave him exercises to do post-op. The case settled for $16,101.02, representing 12.5% loss of his right arm 4% loss of his left arm $3,500.00 in disputed TTD compensation and approximately 2.347% loss of person as a whole. What Is Mommy Thumb

There are tendons that run along the thumb side of the wrist that course through a small compartment adjacent to the forearm bone, radius, that function by extending or pulling the thumb outward. These tendons are covered by tissue, tenosynovium, that bathes the tendon with a lubricant that facilitates gliding of the tendon through this tight space.

Tendons are thick, rope-like structures that connect bone to muscles to allow movement. The synovium is a thin, slippery layer of soft tissue that covers the tendons allowing then to easily glide through the fibrous tendon sheath. The extensor pollicis brevis and the abductor pollicis longus tendons assist in pulling the thumb away from the palm. As these tendons move into the wrist they pass through a tunnel called the first dorsal compartment.

With repetitive use of the thumb or in some cases a specific injury, an increase in inflammatory tissue develops which reduces the space within this compartment leading to an increase in tendon friction which causes pain. De Quervains Tenosynovitis occurs when the lining of this sheath thickens, constricting the tendons.

For decades people have been afflicted with this condition, known as de Quervains tendonitis. In recent years with the development of PDAs or small electronic devices that allow frequent data input in the palm of the hand for texting or emails, this condition has been referred to as Blackberry Thumb.

    What Is De Quervains Tenosynovitis

    Tendons attach muscles to bones. These rope-like structures pull on the bone when the muscles contract creating movement. Tendons lie inside tunnels.

    There are two tendons that generate normal function in the wrist and lower thumb. These tendons slide through the tunnels when gripping, clenching, wringing, pinching, or grasping.

    With de Quervains Tenosynovitis, the tunnel between the thumb and wrist narrows. This can result from the thickening of the tunnel or increased soft tissue around the tunnel. The person experiences pain with thumb movements such as forceful grasping or twisting.

    The primary cause of de Quervains Tenosynovitis is repetitive use of the hands and wrists. Repeating the same motion over time irritates the lining of these tunnels. This causes them to swell and narrow impairing the tendon motion.

    This condition can also result from a wrist or tendon injury. Scar tissue builds up causing the restricted tendon movement. Inflammatory conditions, such as rheumatoid arthritis, can contribute to these problems.

    If this problem isnt treated, the symptoms can increase. The result may be a decrease or total loss of function in the hand and wrist.
