Complex PTSD describes an intense group of symptoms that stem from trauma, often a series of traumas beginning in childhood. Those living with CPTSD struggle specifically with regulating their emotions, a negative sense of self, and difficulties in relationships. These are coupled with the other typical markers of PTSD, like hypervigilance, intrusive thoughts, and avoidance of reminders or triggers.1

CPTSD is seen to be a product of ongoing trauma that alters a persons sense of self, making it more difficult to both identify and treat.

All medications currently available for treating PTSD are prescription medications. They are either part of a class of medications called selective serotonin reuptake inhibitors or selective norepinephrine reuptake inhibitors .

Serotonin and norepinephrine are neurotransmitters. These brain chemicals directly affect mood. Neurotransmitters pass between nerve cells to carry messages around the brain.

After transmitting the message, neurotransmitters reabsorb into nerve cells, a process called reuptake. SSRIs and SNRIs prevent reuptake, allowing more neurotransmitters to flow in the brain.

Brain chemistry is complex, so this explanation is simplified, but healthcare professionals know that affecting neurotransmitter levels can help relieve symptoms of PTSD.

The number and type of ACEs a person experiences can depend on the persons culture, race, ethnicity, religion, and other identifying factors. Social inequities in the U.S. mean that people from historically marginalized groups can experience more ACEs.

Research shows that 61% of Black non-Hispanic children and 51% of Hispanic children in the U.S. have experienced at least one ACE, compared with 40% of white non-Hispanic children and 23% of Asian non-Hispanic children.

In most regions of the U.S., the prevalence of ACEs is highest among Black non-Hispanic children. Overall, the lifetime prevalence rate of PTSD among Black people is than that of other groups.

Social inequities and inconsistent access to health insurance and treatment also mean that people from marginalized groups

ICD-11 states that a clinician must determine that a person meets all the criteria for traditional PTSD before diagnosing complex PTSD.

In addition, the person must show problems with self-regulation, low self-esteem, a sense of shame or guilt related to past trauma, and problems maintaining relationships with others.

Before the WHO updated its diagnostic criteria to include complex PTSD, clinicians may have chosen to diagnose a person with an enduring personality change after a catastrophic experience or with disorders of extreme stress not otherwise specified. The intention is for the new diagnosis of complex PTSD to replace these old diagnoses.

  • Self-harm or self-injury means a person may hurt themselves purposefully due to emotional or mental distress.

Many people with PTSD try to push memories of the event out of their mind, often distracting themselves with work or hobbies.

Some people attempt to deal with their feelings by trying not to feel anything at all. This is known as emotional numbing. This can lead to the person becoming isolated and withdrawn, and they may also give up pursuing activities they used to enjoy.

What Causes Complex Ptsd

CPTSD is caused by a persistent, elevated stress experience. The most common cause is abuse that begins in childhood that puts people at increased risk for subsequent unhealthy relationships. CPTSD is also linked to people who are exposed to long-term political and social turmoil, such as those who live in communities with extreme violence, refugee camps, or war-zones. Multiple traumatic events over time disrupt the brains typical development.

Unfortunately, the vast majority of child abuse perpetrators are a parent or primary caregiver. Abuse is rarely a single incident. When a child is abused by the person who is supposed to love and nurture them, the attachment they form with that person becomes highly insecure, and future relationships are at higher risk for abuse and neglect as well.5

More exposure to trauma often leads to more complex symptoms of PTSD.6 The messages of the traumas become ingrained in the sense of self.

What Treatments Are Available For Ptsd

PTSD is often a lifelong disorder that cant be eliminated completely. But it can be managed in a way that minimizes symptoms, including hyperarousal, allowing you to live your life to its fullest. PTSD is mainly treated with talk therapy done in an individualized, group, or combined setting. Sometimes mental healthcare providers will also prescribe medication. These treatments help alleviate symptoms in several ways:

  • improving your self-confidence
  • giving you a more optimistic outlook on life
  • teaching you coping mechanisms to deal with your PTSD when youre experiencing symptoms
  • addressing other issues related to your traumatic experience, such as other mental health disorders and drug or alcohol abuse

Common types of psychotherapy include:

  • Cognitive behavioral therapy: This type of therapy helps patients recognize thinking patterns that cause their PTSD symptoms, such as negative self-image and thinking a traumatic event will occur again. Its often used along with exposure therapy
  • Exposure therapy: A type of behavioral therapy that helps a patient confront situations and memories that are traumatic in a safe way so that they can learn to better cope with them. Virtual reality programs are often used.
  • Eye movement desensitization and reprocessing : This is a combination of exposure therapy with a guided series of eye movements that help a patient work through traumatic memories and change the way they react to them.

What Can I Expect

To receive medications for PTSD, you will need to meet with a provider who can prescribe these medications to you. Many different types of providers, including your family provider and even some nurses and physician assistants, can prescribe antidepressant medications for PTSD. You and your provider can work together to decide which antidepressant medication may be best for you.

In general, the 4 different SSRIs and SNRIs listed above appear to work equally well for PTSD. Once you fill your prescription, you will begin taking a pill at regular time each day. It may take a few weeks before you notice the effects of the medication. It is important to continue to take it even if you do not notice changes right away. You will meet with your provider every few months or so. Your provider will monitor your response to the medication and change your dose, if needed.

Post-traumatic stress disorder affected an estimated 3.6% of adults in the United States in the last year. Although its often associated with combat veterans, children and adults can be diagnosed with PTSD as well.

PTSD is a mental health condition is a severe form of anxiety disorder that can last for months or years after someone witnesses or experiences trauma. Triggers can cause the trauma to resurface, resulting in extreme physical and/or emotional responses. Some common symptoms that can affect your everyday life include nightmares, flashbacks, anger, irritability, negative thoughts, sleep disturbance, or hyperarousal symptoms. Those with PTSD may even find themselves experiencing anxiety or depression in conjunction.

There are many treatment options for post-traumatic stress disorder, including PTSD medication and various therapy techniques. The most important thing to keep in mind when considering the right PTSD treatment medication or therapy is that since no two people have the same experience and treatment generally isnt a one-size-fits-all plan.

You might need to try different techniques before something works well for you and your symptoms. In the end, finding a mental health professional with experience in treating PTSD is likely going to be your best bet. Whether this means finding the best medication for PTSD or engaging in short-term trauma-focused CBT for PTSD, there are treatment options out there for you.

Eye Movement Desensitization Reprocessing

EMDR is a pioneering trauma treatment that involves eye movements and psychotherapy combined.

Treatment initially focuses on building a foundation of trust between therapist and patient and teaching eye movement techniques.

EMDR allows trauma survivors to briefly concentrate on a specific traumatic memory and process it until it no longer causes disturbance or distress.

When your loved one is unable to do chores, errands, or daily tasks, you might try to help by taking over those responsibilities.

But its often more helpful to encourage them to take steps toward doing these things themselves and offering support when needed.

You can also ask if theres anything specific getting in the way of tasks:

  • If they havent done laundry because they ran out of laundry soap and feel afraid of leaving the house, you could offer to do a grocery run.
  • If they cant prepare meals because a voice threatens them whenever they pick up a knife, you might help them chop a few days worth of vegetables in advance.

You can also offer to help them plan and schedule out weekly responsibilities when you spend time together.

When Should You Seek Help

If youre thinking of hurting yourself, you need to call 911 or your local emergency number right away.

If youre experiencing hyperarousal or other PTSD symptoms, you should see a doctor. Theyll perform a physical exam to make sure no underlying medical disorders are causing your symptoms. They may also perform a blood test, depending on what other physical symptoms you have.

If your doctor suspects youre experiencing PTSD, theyll refer you to a mental healthcare provider, often a psychologist or psychiatrist. Psychiatrists are able to prescribe medication, while psychologists arent.

Other Antidepressants For Ptsd

COMPLEX PTSD (Post-Traumatic Stress Disorder)

Antidepressants that affect the balance of serotonergic and noradrenergic neurotransmission, or which alter serotonin neurotransmission through other mechanisms of action, are also helpful in PTSD. Venlafaxine acts primarily as a serotonin reuptake inhibitor at lower dosages and as a combined serotonin and norepinephrine reuptake inhibitor at higher dosages. It is also a conditionally recommended treatment for PTSD. A typical dosage range is:

  • Venlafaxine : 75 mg to 300 mg daily

All of the antidepressants described above are also effective in treating comorbid major depressive disorder which, depending upon the study, accompanies PTSD about 50 percent of the time. Dose-related elevations in blood pressure have been noted with venlafaxine. It should be used with caution in patients with hypertension.

What Types Of Trauma Are Proposed To Increase The Likelihood Of Complex Ptsd

Originally, proponents of complex PTSD focused on childhood trauma, especially childhood sexual trauma. However, there is abundant evidence suggesting that duration of traumatic exposureâeven if such exposure occurs entirely during adulthood as with refugees or people trapped in a long-term domestic violence situationâis most strongly linked to the concept of complex PTSD. During long-term traumas, the victim is generally held in a protracted state of captivity, physically or emotionally, according to Dr. Herman . In these situations, the victim is under the control of the perpetrator and unable to get away from the danger. Examples of such traumatic situations include: concentration camps, Prisoner of War camps, prostitution brothels, long-term domestic violence, long-term child physical abuse, long-term child sexual abuse, and organized child exploitation rings.

Other Medications For Ptsd

Topiramate is in the anti-epileptic category of medications and is thought to modulate glutamate neurotransmission. There has been recent interest in its use for PTSD. The systematic review that served as the evidence base for the guideline development panel reported moderate strength of evidence for a medium to large magnitude effect for PTSD symptom reduction.

However, the panel concluded that there was insufficient evidence to make a recommendation because the potential side effects/harms for topiramate are greater than they are for SSRI antidepressants. It is not uncommon for patients taking topiramate to note side effects of cognitive dulling. Topiramate has also been found helpful in reducing alcohol consumption in those with an alcohol use disorder, which frequently accompanies PTSD.

Talking Treatments For Ptsd

The National Institute for Health and Care Excellence the organisation that produces guidelines on best practice in health care currently recommends two types of talking treatment for PTSD:

  • Trauma-focused cognitive behavioural therapy . This is a form of cognitive behavioural therapy specifically adapted for PTSD. NICE recommends that you are offered 812 regular sessions of around 6090 minutes, seeing the same therapist at least once a week. See our pages on for more information about this therapy.
  • Eye movement desensitisation and reprocessing . This is a fairly new treatment that can reduce PTSD symptoms such as being easily startled. It involves making rhythmic eye movements while recalling the traumatic event. The rapid eye movements are intended to create a similar effect to the way your brain processes memories and experiences while you’re sleeping. EMDR Association UK – a professional association of EMDR clinicians and researchers – provides lots more information about EMDR on its website. They also have a search function to find EMDR-accredited therapists in your area.

NICE may recommend other talking treatments in future if they are found to help with PTSD, but more research is needed.
