VR5 | National Center for PTSD

If you or a friend or loved one are experiencing PTSD it is helpful to understand the stages of development and healing.

  • Shock StageDid this really happen? Withdrawal, hypervigilance and easily startled are common reactions in the shock stage. Some are able to recognize PTSD symptoms immediately. For others, it may take some time.
  • Denial StageI am fine! Many individuals do not accept any limitation easily. They experience strong feelings brought on by memories of the traumatic event, but will attempt to avoid the difficult emotions. Often, avoidance is done unconsciously. This is an especially vulnerable time when many begin to use use alcohol or other substances to numb difficult emotions or memories.
  • Short Term Recovery StageAn adjustment to everyday life and return to a sense of normalcy is achieved. However, the underlying trauma is not resolved. Nightmares and intrusive thoughts continue to make life difficult. Alcohol and substance abuse may be present.
  • Long Term Recovery StageIndividuals will continue to deal with the after-effects of trauma including anxiety and nightmares. However with treatment, these symptoms can be reduced and eventually resolve. With treatment and support from professionals, family, and friends those with PTSD can return to a calmer and sober life. Life can be good again!
  • Major Shifts In Your Moods Or Thoughts

Veterans with PTSD often have major shifts in their moods, or mood swings, that come on suddenly and without warning. They may also have intense feelings of shame or guilt about events in their military career, which may lead to feelings of unworthiness. The Connection Between Substance Use And Ptsd In Military Veterans

Veterans dont just have higher rates of substance use. They also have higher rates of PTSD and other serious mental health concerns. PTSD is an anxiety disorder that can be triggered in anyone who experiences a traumatic or life-threatening event. Because military personnel are more likely to experience these and to live in high-stress environments, they are more likely to develop PTSD.

Mental health and addiction are not separate experiences. The VA explains the relationship between the two: Some people try to cope with their PTSD symptoms by drinking heavily, using drugs, or smoking too much. People with PTSD have more problems with drugs and alcohol both before and after getting PTSD. Also, even if someone does not have a problem with alcohol before a traumatic event, getting PTSD increases the risk that he or she will develop a drinking or drug problem.6

Those facing PTSD may have experienced trauma as a result of drug use. They may also turn to drugs or alcohol to self-medicate symptoms.

Unfortunately, this short-term solution leads to more lasting side-effects such as the following:

    • Increased numbing of emotions
    • Heightened irritability, hostility and aggression
    • Avoidance of trauma, symptoms and feelings
    • Difficulties concentrating

The longer a person puts off PTSD and substance use disorder treatment, the more complicated these issues become. Neither just goes away on its own, but with professional help, true healing becomes available and accessible.

Moreover, Heroes Mile respects that everyone has a unique experience with PTSD. Some people need different levels of care, while others may find that certain treatment options work best for them.

To further accommodate for a wide range of needs and situations, we offer three different PTSD rehab programs of varying intensities:

    • Inpatient PTSD rehab. Our residential treatment program offers the highest, most intensive level of care. Patients should consider inpatient treatment if they have severe, debilitating symptoms, or if they lack an encouraging, sober home environment.
    • Outpatient PTSD rehab. For individuals who have graduated from residential treatment or require less intensive care from the start, our intensive outpatient treatment program may be an option. Patients in outpatient treatment come on-site for one to two hours each day.
    • Partial hospitalization. If someone has moderate symptoms but is mentally stable, and if they have a safe home life, they may opt for our partial hospitalization program. These patients commute to the facility for up to five hours each day, then return home at night.

The Signs And Symptoms Of Ptsd

For many Veterans, the signs of their PTSD go unnoticed. They simply think that they are having a hard time adjusting back to non-military life, or they may ignore their symptoms due to concerns about the stigma sometimes associated with seeking mental health care. PTSD is a complex condition because it can cause a range of symptoms that can change from situation to situation, or even day-to-day. However, specialists have identified specific symptoms that are specific to PTSD:

    What Is Ptsd Rehab

Though it can feel like a pervasive, unyielding illness, PTSD is not untreatable. With the proper resources and support, anyone can overcome the symptoms of PTSD and transition back to everyday civilian life.

PTSD rehab stands as one of the most effective methods of making that happen. It introduces individuals to a plethora of helpful tools and skills they can use to cope with their traumatic memories in healthy ways.

This is accomplished through several different techniques. Some of the treatment options utilized at a PTSD treatment program include:

    • Career development
    • Discharge and aftercare planning

Each facet of PTSD rehab treatment exists to fulfill a particular goal or meet a specific need. For instance, EMDR therapy helps patients work through their traumatic memories and reach a point where they can speak about them without getting triggered. Group therapy boosts communication skills, while family counseling can rebuild bonds and increase emotional awareness.

Exposing patients to a broad range of therapeutic techniques sets them up for the highest chance of success. As mentioned before, PTSD can impact every area of someones life. Therefore, it stands to reason that PTSD treatment needs to address just as many concerns.

    Implementation Of Partial Pe Protocol

    Cognitive-Behavioral Conjoint Therapy for PTSD

While the reasons for partial use primarily involved perceived structural barriers, such as lack of time and resources, it also reflects programs preferences for other psychotherapies or a belief that elements of PE, such as in vivo exposure, were effective and complementary to other programming provided.

    1. In vivo group

Some programs implemented only elements of the PE protocol, namely in vivo exposure. Most of these programs offered groups in which providers assisted patients in systematically creating in vivo hierarchies and approaching situations, people or places that were previously avoided due to trauma. Separately or as a group, veterans participate in in vivo activities within areas of the medical center or in the community . Following in vivo assignments, veterans are given time in group to discuss the experience, provide support for one another, and review SUDs levels. One program offers one week of group psychoeducation on in vivo exposures and the following week on outings to complete actual in vivo assignments.

Prevalence Of Substance Abuse In The Military

Active duty military personnel have unique circumstances and personal experiences that involve heightened stress levels and trauma. Psychological distress can be a risk factor for substance use. Alcohol and drugs can provide temporary feelings of relief or self-medication for stress, trauma and PTSD symptoms.

Even during active duty, members of the military have issues with substance abuse at different rates than the general public. The Health-Related Behavior Survey Active Duty Service Members estimated that as many as 84.5 percent of active-duty military personnel were current drinkers in 2011 with 11.3 percent considered to be problem drinkers.3 This is higher than the national average, as the National Institute on Alcoholism and Alcohol Abuse estimates that approximately 7.2 percent of American adults over the age of 18 in 2012 were classified as having an alcohol use disorder .

Illicit drug use rates tend to be lower in the military, but this may be because of the militarys zero-tolerance policy and the hesitance of active members and veterans to report use. As in all areas of the population, prescription medication use and abuse rates remain high.

The National Institute on Drug Abuse shares, Pain reliever prescriptions written by military physicians quadrupled between 2001 and 2009 to almost 3.8 million.5 This translates to high substance abuse and addiction rates within the military as well.

How Common Is Ptsd With Veterans

It has been estimated by the VA that 11 to 30 percent of Veterans suffer from PTSD, with varying generational prevalence rates. This makes Veterans at least twice as likely to end up with a PTSD diagnosis as the civilian population. As a member of the armed services, individuals are likely to experience unique circumstances that the civilian population wont. These experiences account for the increased prevalence of PTSD in Veterans. Things like deployments, combat, military sexual trauma, and the increased stress and pressure of certain specialties within each branch all combine to increase the chances of developing PTSD symptoms.

Why Does Substance Use Start And Continue

Being away from home and family and immersed in a high-stress environment may encourage military members to engage in substance use. Many substances, including alcohol, are central nervous system depressants.

These types of drugs can temporarily provide the following:

    • Feelings of calm
    • Emotional numbness

Its easy to see why drinking and drug use seem appealing at first. Drugs and alcohol increase the production of dopamine, a chemical messenger related to pleasure and reward, and further encourage the use of these substances.

However the more often a person drinks or uses, the more likely dependence and addiction become. Chronic use or abuse of a substance creates a physical dependency. The brain and body begin to expect the substance and to feel wrong, off or ill without it. Drunk or high becomes normal. At the same time physical tolerance begins to develop. Individuals will need more or higher doses to create the same effects. Individuals will experience withdrawal symptoms when they stop using or even in between doses. These further reinforce continued use.
